The Ledge Light Health District ACHIEVE New London initiative has launched a campaign to survey New London Residents. The survey 2009 New London Healthy Resident Survey is being distributed citywide in an effort to include all citizens. The purpose of the survey is to engage the community in a process of identifying and selected environmental and policy changes to prevent chronic disease.  Attached you will find copies in both English and Spanish of the survey.
